The content and the credentials are in very good shape. What is costing points is mechanical: one dead button on the Assisted Living page, no question markup anywhere, and a tag stack that has grown layers over the years.

Responsive theme with a working text size control. Hero and page header images are single fixed files with no srcset, so a phone downloads the 1200 pixel wide version.

Conversion path

The one button in the body of the Assisted Living page does not go anywhere

Directly under the paragraph that explains the Small House model of care, there is a button that reads Request information. Its link target is the page it already sits on, so clicking it scrolls the reader back to the top of the same page. The visitor who just read the best three paragraphs you have written about assisted living has nowhere to go but the main menu.

Check it: open forestduke.org/assisted-living/, scroll to the photo under the care paragraphs, click Request information. The address bar becomes /assisted-living/# and the page does not change.
AEO, answer engines

You already wrote the answers. Nothing on the site says they are answers.

The Assisted Living page carries three expandable panels titled Cost and Fees, Qualifications, and Touring. Those are exactly the three things a daughter in Raleigh types into a search box. Semrush shows 830 of your ranking keywords returning a People Also Ask box and 208 returning an AI Overview, and you hold zero featured snippets and zero FAQ results against any of them. There is no FAQ page anywhere on the site and no question markup on the panels you do have, so an answer engine reading your pages has no structured question to quote. It quotes a directory site instead.

Check it: on forestduke.org/assisted-living/ the three panels under Next Steps are the content. Then view source on any page and search for FAQPage. There is no match on the site.
SEO and structured data

Every interior page has two h1 headings, and the structured data says almost nothing about you

The theme prints the page title twice, once in the banner image for desktop and once again below it for phones, so each page ships two competing h1 elements with the same words. Separately, the Yoast block on each page describes a generic WebPage, a BreadcrumbList and a WebSite whose description field is an empty string. There is no Organization and no LocalBusiness block, which means the 47 acre campus, the Pickett Road address, the CARF and CCAC accreditation and the Newsweek ranking on your Awards page are invisible to the systems that assemble answers about Durham retirement communities. Your best proof is sitting in text that machines do not read as proof.

Check it: view source on forestduke.org/history/ and search for page-title. It appears twice. In the same source, search yoast-schema-graph: the graph contains WebPage, BreadcrumbList and WebSite, and "description":"" on the WebSite node.

3A 90 day fix sequence

Days 1 to 10 Point the Assisted Living button at a short assisted living inquiry form. Add the same button after the Cohen Center paragraphs and at the foot of the Amenities page. Three of your strongest reading pages currently end in a menu. This is the cheapest conversion change on the site.
Days 10 to 25 Turn the Cost and Fees, Qualifications and Touring panels into a real question and answer block with FAQPage markup, and build one Assisted Living FAQ page from the questions your sales counselors answer on the phone every week. 830 keywords already return a question box. You are eligible for none of them today.
Days 20 to 35 Add an Organization and a LocalBusiness block naming The Forest at Duke, Inc., the 2701 Pickett Road address, the two phone numbers, CARF and CCAC accreditation, and the award list. Puts your credentials where the 381 local pack keywords and the 208 AI Overviews can read them.
Days 30 to 45 Remove the duplicate h1. Keep one heading and hide or show it with CSS rather than printing it twice. One clear heading per page. A ten line theme change.
Days 40 to 60 Audit the head. The legacy Universal Analytics property still fires on every page alongside Google Tag Manager, and two different Meta Pixel identifiers fire on the same pageview. Consolidate into one measurement plan. Removes double counting from paid reporting and takes third party requests out of first paint.
Days 55 to 75 Serve responsive image sets for the page header and hero images, and self host jQuery and the webfont stylesheet. A phone on a Durham cell connection stops downloading desktop sized JPG files from three hosts.
Days 70 to 90 Stand up a reporting view that joins inquiry source, residency type requested, tour attendance and move in, using the questions already on your contact form. Your form asks how people heard about you and which level of living they want. Nothing currently reports those two fields against move ins.
